Focus on pertinent positives and negatives. Skip irrelevant details—document only what matters for the history and physical examination.
Complete your data as thoroughly as possible. The more complete your entries, the more accurate and clinically useful your history output will be. Incomplete data leads to incomplete drafts — fill in as many fields as relevant to your case.
Use the "Draft H&P" button once your fields are filled in. It will format your entries into a structured clinical narrative ready for any AI assistant to refine into your final history draft.

How to Use ClinDocDraph
Choose a Specialty & Pay via QRPh
Click a specialty (e.g., OB-GYNE, Pediatrics, Surgery) → Pick a bundle → Scan the QRPh code → Pay → Upload your receipt with your nickname.
Retrieve Your Key (within 24 hours)
Once your payment is verified, go to 🔍 Retrieve My Key in the sidebar and enter your nickname to get your activation key.
Activate on This Device
Go to 🔑 Activate Key, enter your nickname AND activation key — both are required. Your license is locked to this device/browser only.
Start Documenting!
