To our fellow colleagues,
We've all had those shifts. It's late, the ward is finally settling down, but you're still stuck at the computer. You know exactly what's going on with your patient, but actually typing out a solid HPI or going through the "Big 6" for the tenth time that day feels like a mountain you just can't climb anymore.
We built this app because we were tired of being professional typists instead of doctors. We wanted something that actually understood how we think during rounds — the way we look at polypharmacy, the specific flow of a Peds or OB-GYNE note — without having to deal with some complicated system that asks for your email or stores your data.
We kept it simple for a reason: It actually speaks our language. It's built around the history and physical examination templates we use every day. It's meant to be a drafting assistant that gets the "boring" part of charting out of the way.
Privacy is our priority. We know how it is with hospital data. That's why there are no accounts, no emails, and nothing is stored. You just get your draft done, copy it, and you're good to go.
Our goal isn't to do the job for you, but to give you back a bit of your time so you can focus on your patients and hopefully get home a little earlier.

1Jurisdiction & Administration
This tool is a localized utility administered by licensed Filipino physicians. It is intended for professional use and does not specifically target or serve any particular civilian population.
- •No Physician-Patient Relationship: Use of this tool does not establish a clinical relationship with the Admin Team.
- •No Contact Policy: In adherence to our strict anonymity and security protocols, no email or personal support is provided.
- •Technical Classification: This application is a Clerical Drafting Utility only. It is not an Electronic Medical Record (EMR) system, a Telemedicine platform, or a Diagnostic Software. It does not store patient data and is not intended for the permanent retention of medical records as defined under DOH Joint Memorandum Circular No. 2020-0016.
2Non-Diagnostic Disclaimer
This tool is a clerical formatting utility. It does not generate diagnoses, clinical assessments, or treatment plans — it only restructures raw text into a standardized history and physical examination format. Assessment & Plan sections, where present, are user-authored fields only; the app does not generate or suggest clinical plans. All clinical conclusions are the independent work of the user.
Authentication Notice: All drafts generated are "Electronic Data Messages." The responsibility for the integrity, reliability, and final authentication (signature) of these documents rests solely with the user upon transferring them to their official institutional records.
3Human-in-the-Loop Requirement
All clinical content is authored solely by the user. This utility only reformats the data you provide. Users must perform a "Human-in-the-Loop" review — the output is a draft only and must be manually edited, corrected, and finalized by the physician before being entered into any official medical record.
- •Licensed Physicians: You remain the final clinical authority and are solely accountable for the accuracy, safety, and legality of any medical record generated or finalized using this web app.
- •Medical Students & Trainees: All clinical drafts produced must be manually validated by a supervising licensed physician before implementation.
4Data Privacy & NPC Compliance
Users are strictly prohibited from inputting direct identifiers (Full Names, Exact Birthdays, or Specific Addresses). You must use Alias Names only (e.g., "Patient AKA X") to remain compliant with the National Privacy Commission standards.
Not a Medical Record System: This tool is not integrated with PhilHealth, PhilSys, or any National Health Data systems. Use for temporary drafting only.
5Limitation of Liability
The ClinDocDraph Admin Team and the ClinDocDraph web app are not liable for technical errors, "hallucinations," or logical inconsistencies in generated drafts. By using this service, you agree to indemnify and hold us harmless from any legal claims, malpractice suits, or regulatory actions arising from the use of these drafts or the failure to manually verify them.
Legal Recognition of Output
Any electronic data message or document formatted by this utility is considered the functional equivalent of a written document. However, the Originator (the User/Physician) is solely responsible for ensuring the integrity, reliability, and authentication of the final output. The administration acts strictly as an intermediary service provider and does not "author" the clinical content.
6🔒 Enhanced Security: Auto-Purge Technology
To comply with NPC standards, this app utilizes Auto-Purge. Once you copy your draft, all inputs and outputs are immediately wiped from your local browser memory. We don't just "not store" your data; we ensure it no longer exists.
- •Zero Persistence: This application does not store, cache, or transmit inputs to any database.
- •Administrative Anonymity: To prevent unauthorized access and maintain security, no personal contact information or email addresses are stored or provided.
7Payment & Infrastructure
The maintenance fee collected is for the use of the data-processing infrastructure and API costs. It is not a professional fee for medical services or consultations.
Local Payment Gateway: This tool utilizes QRPh as its localized payment gateway to support server maintenance. The Filipino origin of this tool ensures that its organization templates are optimized for local clinical practice settings.
⚠️ No Refund Policy
All payments are final and non-refundable. If your payment has been made but your key has not yet been reflected in the system, please wait up to 24 hours for manual verification. Delayed reflection does not constitute grounds for a refund. By completing payment, you agree to this no-refund policy.
8Scope of the Drafting Assistant
ClinDocDraph is a History-taking drafting assistant only. It is not a complete clinical documentation solution and does not cover all sections of a full clinical record.
- •Format Variability: History and Physical Examination formats vary between institutions. The templates provided here are general guides and may not match your institution's exact required format.
- •Incomplete by Design: This history format is not 100% complete. Users are expected to supplement the output with institution-specific fields, additional clinical findings, and professional judgment.
- •AI Hallucination Warning: AI models can occasionally generate inaccurate, fabricated, or misleading content. Always critically review every draft output before use in any official or clinical document.

Zero Persistence: To protect sensitive clinical data, this application does not store, cache, or transmit inputs to any database.
Mandatory De-identification: All users are required to use Alias Names only.
Administrative Anonymity: To prevent unauthorized access and maintain the security of the Licensed Physicians administering this tool, no personal contact information or email addresses are stored or provided.
Technical Support: This tool is a non-commercial, physician-led initiative. To ensure 100% user privacy and administrative security, we do not collect user emails. As such, individual account support is not provided.
